Composed to be said before surgery to remove the uterus and/or ovaries. It can also be said at the time of a mastectomy, or any surgery that may lead to menopause. The original Hebrew is below.

Compassionate God,
At this time, as I stand before you about to part with {my womb and one of my ovaries}*
In order to protect my health,
I entreat you to enable the doctors to treat me professionally, sensitively and with respect.
I offer thanks to You for putting me in their good hands.
Please, God, open the gate [of mercy] for me at this when my gates are closing.
Allow me to feel complete, womanly,
Even though, of all the organs, it is my {womb}* that is missing.
I give thanks to You for the years in which my organs functioned as they are supposed to, and did the Will of their Creator,
For the years of fruitfulness/fertility, for the months of pregnancy in which my womb carried my children.
I give thanks to You for allowing me to part from these organs with a whole heart.
May this be the beginning of a blessed and compassionate stage of life,
a time of health and healing for me and all my loved ones.
Compassionate God,
At the time when my {womb (rehem)}* is being removed from my body, please increase and expand Your mercy (rahamecha) on all Your creatures,
on the members of my household, on our families,
upon your people Israel,
And in particular, on all the women who ask for the fruit of the womb
And on women who are seeking salvation from their suffering,
Please show mercy (rahamim) on them and send them healing of body, soul and heart.
I pray that I will always feel wrapped in your outstretched arms, Merciful Father

אֵל מָלֵא רַחֲמִים
בְּשָׁעָה שֶׁנִּצֶּבֶת אֲנִי הַיּוֹם לְהִפָּרֵד {מֵרַחְמִי וּמֵאַחַת מִשַּׁחֲלוֹתַי}*
עַל מְנַת לְהֵיטִיב עִם בְּרִיאוּתִי,
מְבַקֶּשֶׁת אֲנִי מִמְּךָ שֶׁהָרוֹפְאִים יְטַפְּלוּ בִּי בְּמִקְצוֹעִיּוּת, בִּרְגִישׁוּת וּבְכָבוֹד.
מוֹדָה אֲנִי לְפָנֶיךָ עַל כָּךְ שֶׁאֲנִי מְסוּרָה בִּידֵיהֶם הַטּוֹבוֹת.
אָנָּא ה' פְּתַח לִי שַׁעַר בְּעֵת נְעִילַת שַׁעַר.
תֵּן לִי לְהַרְגִּישׁ שְׁלֵמָה, אִשָּׁה,
גַּם כְּשֶׁמִּכָּל רְמַ"ח אֵיבָרַי חָסֵרָה {הָרֶחֶם}*.
מוֹדָה אֲנִי לְפָנֶיךָ עַל שָׁנִים שֶׁבָּהֶן תִּפְקְדוּ אֵיבָרַי כְּסִדְרָם וְעָשׂוּ רְצוֹן קוֹנָם,
עַל שְׁנוֹת פִּרְיוֹן, עַל חָדְשֵׁי הֵרָיוֹן בָּהֶם נָשְׂאָה רַחְמִי אֶת יְלָדַי.
מוֹדָה אֲנִי לְפָנֶיךָ עַל הַתְּחוּשָׁה כִּי נִפְרֶדֶת אֲנִי מֵאֵיבָרִים אֵלֶּה בְּלֵב שָׁלֵם.
מִתְפַּלֶּלֶת אֲנִי שֶׁיְּהֵא זֶה פֶּתַח לִתְקוּפַת חַיִּים מֵיטִיבָה וּרְחוּמָה,

בְּשָׁעָה שֶׁאֲנִי מְחַסֶּרֶת מִגּוּפִי אֶת {רַחְמִי}* שֶׁלִּי, אָנָּא הוֹסֵף וְהַגְדֵּל רַחֲמֶיךָ עַל בְּרִיּוֹתֶיךָ,

וּבִפְרָט עַל נְשׁוֹת יִשְׂרָאֵל הַמְּבַקְּשׁוֹת פְּרִי בֶּטֶן
וְעַל נָשִׁים הַמְּבַקְּשׁוֹת גְּאֻלָּה מִיִּסּוּרֵיהֶן,
אָנָּא הִתְמַלֵּא רַחֲמִים עֲלֵיהֶן וּשְׁלַח לָהֶן רְפוּאַת גּוּף, נֶפֶשׁ וָלֵב.
מִתְפַּלֶּלֶת אֲנִי שֶׁאַרְגִּישׁ תָּמִיד עֲטוּפָה בִּזְרוֹעוֹתֶיךָ הַמּוּשָׁטוֹת, אָב הָרַחֲמָן.
